Annual Reporting

Due to state compulsory attendance laws, Deer Chase Academy must maintain attendance records for all enrolled students. Every student is required to attend no less than 180 days of school within a one year period from the starting date of his school year. Please use the form below to record your child's attendance. DCA must have a copy of this form by no later than one year plus one week from your starting registration date, or your child's enrollment will be considered terminated. This form may be downloaded, printed, completed and signed and then sent to our office in one of two ways.

You may mail it to the following address:
Deer Chase Academy
338 Erie Drive
Kissimmee, FL 34759

Or you may scan a copy and e-mail it to: registrar@deerchaseacademy.com.
(Please put your student's name in the subject line.)

Attendance Record - This form requires Adobe Reader, which may be acquired freely at: adobe.com.

Did you enroll in Deer Chase Academy in the middle of an academic year after pulling your child from a traditional school? The days he attended at school count toward his total attendance days. In other words, if he already attended 65 school days at the local public school before withdrawing and registering with DCA, he needs to then complete 115 days of homeschooling to meet his year's worth of required days. Please make sure to add a written note on his attendance report indicating which days were from a previous school setting. Also, remember, his year started the day he began school at his previous school, not with DCA. So, if his start date at school was August 15th, his annual attendance report will be due no later than August 22nd of the following year, no matter what date he registered with DCA.
