**In order to register as a Deer Chase Academy student, you must be a resident of the state of Florida.
Please fill out the following form for each student.

This form may be mailed to:

or sent as a scanned e-mail attachment to: registrar@deerchaseacademy.com.
In addition to the registration form, you must submit the following:
1. A copy of your child's birth certificate. (Does not need to be notarized.)
2. A school-entry physical dated within the last year, or Religious Waiver.
3. A copy of your child's current immunization records, or Religious Waiver.
4. All appropriate Fees.
